Rates:   $125.00 per initial  evaluation and $100 per 50-minute session.  All payments are due prior to or at the time of the session.  There may be discounts if you qualify.  We  will invoice you each month so you can submit your billing to your insurance for possible reimbursement.  Please check our the following questions with your insurance company!


Insurance:  We are an in network with just a few insurance companies.  Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Out-of Network Benefit Option:
If services are not covered by your health insurance or employee benefit plan, you may have an out-of-network rate. Please check your coverage carefully by asking the following questions:

• Do I have out-of network mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?
• What forms do I need to submit for out-of-network benefits?  
• What is the claims address to submit monthly invoices? 

Payment:
Cash, check are due at the time of service.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
